For
New Patients
Welcome!
Your First Visit is all
about you –
Understanding how we can best
help you …
Taking care of any urgent needs …
And gathering information that will enable us
to provide safe, effective, state-of-the-art dental
care.
Your first appointment will
consist of a "get acquainted" discussion
of your dental needs and decision as to which
x-rays may be right for you at this time. Then
you will have a dental exam, and digital x-rays
if indicated. Dr. Loshin will check for many things
including cavities, gum disease, broken teeth
and oral cancer. We may magnify your teeth with
a 50x video camera. You will be able to see these
images on a screen above the dental chair, if
you like. You will experience very individualized
attention. Expect to spend about one hour for
your first appointment.
If an urgent, need such as
a toothache or cosmetic problem exists, we will
address this problem first, and may postpone the
full dental exam until a later visit.
Second Appointment
At the end of the first visit
you will make an appointment for a consultation
and teeth cleaning. At that second visit we discuss
any necessary treatment, including benefits, costs,
and payment arrangements. Then you will see one
of our dental hygienists for a teeth cleaning,
using the latest ultrasonic equipment, for a thorough,
but gentle experience. Allow up to 90 minutes
for this visit.
At future visits for routine
preventive care, all x-rays, cleaning and exams
are done in a single visit of about an hour. But
at your very first new patient visit there is
not time to also clean your teeth.
Payment for dental services
We request payment at time
of service for-
Patients without dental insurance
Patients who do not
have their insurance information
Billing arrangements
for subsequent visits will be outlined at the
time of consultation. If you have dental insurance
please bring your insurance card.
New Patient Forms
Please complete the 3 New Patient
Forms as accurately as possible. We are interested
in helping you with your program of health and
well-being. Good dental health should be an important
part of that program.
It is necessary as part of
any complete examination to have a basic understanding
of your general health. Important links between
our dental health and general health are increasingly
recognized by the medical profession. The information
you provide will assist in obtaining an accurate
appraisal of your oral health, so that we can
better serve you. This material, of course, will
be held confidential.
